I understand that I must turn in my Membership card to cancel my membership. Administration fees will be applied on overdue accounts. I warrant that I am physically and mentally well enough to proceed. I am aware that the equipment being used in the Gym and the use of it by me is a dangerous undertaking and in undertaking such activities I do this at my own risk. I am also aware that it is a condition of admission to membership of Focus Well-being & Fitness that this Centre, its directors, instructors. members, servants and agents are absolved from all liability whatsoever arising from injury or damage however so caused (where fatal or otherwise) arising out of membership of the Centre or in any way whatsoever due to any negligent act, breach of duty, default and/or admission on the part of this Centre its directors, instructors, members, servants or agents.
